Geriatric Assessment and Family Support
This service is performed in the office, or in the home of the older adult who needs an evaluation of cognitive and functional skills. The evaluation takes into account an inventory of past medical and surgical history, psychiatric and life skills, and values the individual’s contribution to society in younger years. A variety of objective testing is performed including a brief cognitive and physical assessment.
Assessment of daily and independent functioning, and cognitive needs is integral for the safety of the older adult. Evaluation of these aspects of care is completed during the initial visit.
Discussion of findings with the individual and the caregiver is shared with recommendations for care and management in the home, strategies and resources available to help the individual continue to find purpose in waking every morning, leaving the home setting to remain social and interact with others, and returning later in the day. It also provides the caregiver with ideas of how to manage and redirect the older adult to prevent injury to themselves or others, while prompting creative activities to assist the individual to feel valuable as an integral member of the family unit.
